Registration Form








    Policy for Changes/Cancellations/Transfers: Monies paid may be refunded, less a twenty five percent processing fee, up to ninety calendar days prior to a program or course for which Canadian HR Academy has been contracted. Effective ninety calendar days immediately before delivery of the program’s or course’s scheduled date, monies that have been paid are not refundable and will be applied to a future program or course, if taken within one hundred and eighty calendar days of the cancellation date of the original program or course. Transfer to another offering of the training is not available effective fifteen business days prior to the scheduled start date of the offering of the program or course for which a person or a group was originally registered. Upon acceptance of a proposal that was issued by Canadian HR Academy and or upon signing and sending in for processing a registration form that was issued by Canadian HR Academy, whether through email or on our website, the total amount of our fee as reflected on the invoice that is sent to you, is due and payable within three business days of its date. Upon acceptance of our proposal and or signing of our registration form, you have entered into a contract with us. Upon acceptance of our proposal and or signing of our registration form, whether or not a date has been set for the training, our cancellation fee is fifty percent of the amount set out on the associated invoice for the registration form or the associated invoice for the proposal. There will be an $85.00 fee charged for each cheque that is returned by the bank. This policy is an integral part of Canadian HR Academy’s registration form. By initialing the registration form you have signed and accepted it as well as this policy. I agree that I will not operate a vehicle of any kind during Canadian HR Academy training sessions.
